Enrollment & Orientation

We serve children from ages 6 weeks through 5 years old. To enroll your child in our program.  Your child’s space in our program will only be secured once all the following is completed:

  • Registration Form

  • Consent and Release Form

  • Emergency Contacts and Pick up Authorizations Form

  • Child’s Physical form and Immunization Records

  • Child and Adult Care Food Program Form

The Staff at Florence M. Lakin Child Development Center will reach out to meet with you to discuss financial responsibility, sign care contract, and go over our parent handbook.
